The unit/household social worker, in coordination with the interdisciplinary team, assists residents with their psychosocial needs to promote optimal resident satisfaction, well being, and ability.
1. Assists in pre-admission, initial and ongoing resident assessment by way of verbal and written intake, review of written records and observation. Obtains social and medical information from residents and/or their families and the housing manager to be used in the development of the resident’s plan of care, respecting resident’s choices, privacy, dignity, rights and independence.
2. Coordinates information to residents and family members explaining facility and departmental policies and procedures, the functions and services provided by Bethany and answer questions they may have, provide financial information, and referrals to various programs and services within the facility and community.
3. Communicates necessary information regarding the resident’s psychosocial dynamics by medical record documentation, progress notes, discharge planning and verbal communication with physician, nursing staff, department supervisors and other interdisciplinary staff within the facility.
4. Completes required documentation (initial social assessment, social history, MDS sections, discharge plan of care, level of care screening, advanced care planning progress notes, social service behavior notes, social service interval progress notes, depression assessments and elopement assessments).
5. Schedules and facilitates care plan conferences with the interdisciplinary team and resident/resident representative.
6. Actively participates in the care planning process representing the psychosocial needs of the resident, how staff can meet these needs, reviewing life roles, and assisting in identifying new skills and interests and solicits resident/resident representative input and involvement into the resident’s plan of care.
7. Strives to ensure resident rights are understood by the resident and family and advocates for the protection of these rights. The unit/household social worker strives to develop an increased understanding and sensitivity to resident rights throughout Bethany and must be able to address both observed and reported abuses of rights and recommend and provide (when determined appropriate by the interdisciplinary team) interventions when necessary.
